Basic Life Insurance

This benefit provides you with automatic life insurance coverage.

You do not have to make any choices regarding Basic Life as it is 100% paid by Teva Canada.

Through the Choices Benefit Plan, you automatically receive Life Insurance coverage equal to Two (2) times Annual Salary (maximum coverage $1 million)

 

Optional Life Insurance

In addition to the Basic Employee Life Insurance the Choices Benefit Plan offers you the ability to select additional coverage in the event of your death.  You are able to choose from $10,000 to $250,000 of coverage in units of $10,000.

The selection of some options will require you to provide evidence of your insurability (EOI). A pop up will appear if EOI is required. At the end of the enrollment after you have hit the "Confirm" button you will be provided an opportunity to print out the required EOI forms. The form is required by the insurance company to determine whether you satisfy their health requirements. While awaiting the approval the system will automatically enroll you in an option that does not require EOI. Once your approval has been received your plan will be brought up to date and the coverage will be effective from that time forward.
